Nurse Practitioner (NP), Physician Assistant (PA), Job, State Traveler, Full-Time, Minnesota - Saint Paul, MN

Hueman is hiring Nurse Practitioners and Physician Assistants to complete Health Risk Assessments (HRAs) for Medicare and Medicaid members. This position is a full-time state traveling opportunity in Minnesota. Nurse Practitioners will be required to travel the entire state as needed completing 30 assessments per week/6 assessments per day. You will be provided with a company car, hotel, and travel reimbursement as well as a meal stipend.

NP/PA Job Education and Experience Requirements:

Masters Degree OR commensurate experience and satisfactory completion of NP/PA license
Active State NP/PA license
AANP, ANCC, or NCCPA board-certification
Current BLS certification
12 months experience as an NP/PA in geriatric, adult, or family practice

Qualified providers seeking this Health Risk Assessment (HRA) opportunity, will have the special opportunity to interact with the member in a more intimate setting. In contrast to a typical clinic visit, the members are in the comfort of their own home, allowing the provider to connect with them on a more effective level.

Providers performing these health risk assessments are not required to treat or prescribe in the home. This is strictly a wellness exam including a comprehensive physical assessment, review of medical history, perform medication reconciliation as well as some basic patient education.
