Job Title: Advanced Practice Clinician - Nurse Practitioner/Physician Assistant

Location: Minnesota Counties (Anoka, Hennepin, Carver, Scott, Dakota, Washington, Ramsey)

About the Role: The Senior Community Care (SCC) program is a longitudinal, integrated care delivery model coordinating the clinical care of members in facility settings. As an Advanced Practice Clinician (APC) within SCC, you will provide care to high-risk health plan members as part of an interdisciplinary team.

Primary Responsibilities

• Conduct comprehensive assessments
• Manage acute and chronic medical and behavioral conditions effectively
• Ensure accurate documentation, including ICD-10 conditions
• Collaborate with the interdisciplinary care team
• Lead advanced illness and advanced care planning discussions
• Provide counseling and education to patients and caregivers
  
  

Required Qualifications

• Active and unrestricted license in the state of residence or ability to obtain by start date
• Education:
• Nurse Practitioner (NP): Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Physician Assistant (PA): Graduate of an accredited Physician Assistant degree program
• Board certification through a national board:
• NP: Certification through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with specialization in Family, Adult, Gerontology, or Adult-Gerontology Acute Care Nurse Practitioner
• PA: Certification by the National Commission on Certification of Physician Assistants
• Active DEA licensure/prescriptive authority
• Physical ability to perform job tasks, including lifting up to 30 pounds, navigating stairs, and performing assessments
• Reliable transportation
• Ability to gain a collaborative practice agreement, if applicable
• Full COVID-19 vaccination
  

Preferred Qualifications

• 1+ year of clinical experience (long-term care setting preferred)
• Experience meeting the needs of patients with complex medical, behavioral, social, and functional issues
• Understanding of geriatrics, chronic illness, advanced illness, and end-of-life care
• Proficient in documenting medical information using EMRs
  
  

Details

• Schedule: Monday - Friday, 8 AM to 5 PM
• Travel: Between five facilities (Benedictine, Edenbrook of Edina, The Estates at Greeley, The Estates at Linden, Crest View Lutheran Home)
• Dress Code: Medical attire (no specific scrub color required)
